New Patients

New Patients:

If you're preparing for your child's first visit, you can do a few things to help expedite the appointment:

1. Contact your insurance company to make sure that our office is a participating provider on your plan.
2. We need to have a copy of your child's immunizations, their last physical, and growth charts PRIOR to scheduling any appointments.
3. Once we have a copy of the records in our office, we can register your child and schedule their first appointment.
4. To complete the registration process, please fill out ALL of the forms below and remember to bring them in to your initial visit.

Newborns:

Congratulations on your growing family & thank you for choosing our practice! We look forward to providing quality care for your newborn. We know that this is a happy, but stressful time for your family and we hope to help ease some of that stress. Here are a few helpful tips for you:

1. Contact your insurance company to make sure that our office is a participating provider on your plan.
2. Once you have been discharged from the hospital, contact our office to make your first appointment.
3. To complete the registration process, please fill out ALL of the forms below and remember to bring them in to your initial visit.
4. You MUST bring your discharge paperwork to your first appointment.
5. Whether you are adding your newborn to your own insurance policy or getting them an individual policy, you only have 30 DAYS to do so. If you do not add the baby to your insurance before 30 days, they will not have active coverage from their date of birth forward. This will result in you receiving large bills both from the hospital and from the doctor's office or specialists they may see.
